Key Responsibilities
? Conduct diagnostic evaluations, diagnoses, psychotherapy, treatment planning, and consulting services, both in-person at Skilled Nursing Facilities and remotely using our Telehealth software.
? Complete documentation efficiently in our electronic medical record system (Athena I) from the comfort of your home or office.
? Adhere to agency policies and procedures to ensure the highest standard of care.

Requirements
? Master's degree in Social Work, Counseling, or a related field, with Arizona licensure as an LCSW, LMSW, LMFT, LAMFT, LPC, or LAC.
? National Provider Identifier (NPI).
? Professional Liability Insurance.
? Credentialing with Medicaid/Medicare is a plus!
? Some facilities may require up-to-date vaccinations or appropriate exemptions.
? Tuberculosis (TB) Test.
? CPR/AED/First Aid Certification.

Skills
? Basic computer proficiency.
? Excellent written communication abilities.
? Attention to detail and strong organizational skills.
? Strong interpersonal skills and ability to work under pressure.
? Fluency in Spanish is a plus!
