Hospital Liaison-Weekends-Remote Qualifications & Responsibilities...
? Foster collaborative relationships with hospital discharge planners and medical professionals.
? Conduct assessments and evaluations of potential residents in hospital settings.
? Coordinate smooth transitions for patients from hospitals to our nursing home.
? Educate healthcare providers and community partners about our facility's services and capabilities.
? Act as a liaison between the nursing home and external healthcare entities.
? Bachelor's degree in healthcare administration, business, or a related field (preferred).
? 3-5 years Previous experience as an Admissions Director or Hospital Liaison in a skilled nursing/LTACH, or acute rehab hospital desired.
? Strong communication and interpersonal skills.
? Knowledge of healthcare regulations and standards.
