What to Expect (Job Responsibilities):
• Conduct outbound calls to newly enrolled VA Medicaid members to establish first contact and complete health screenings
• Assist members in navigating and maximizing their health plan benefits, including scheduling appointments and arranging transportation
• Provide outreach to locate and support disconnected members with special needs
• Conduct research to contact members that Molina Healthcare has been unable to reach
• Participate in ongoing or project-based activities requiring extensive member outreach

What is Required (Qualifications):
• High School Diploma or GED
• Minimum 1 year of experience working with underserved or special needs populations
• Must have a valid driver's license with a good driving record and reliable transportation
• Experience in a healthcare-related field is a plus
• Ability to collaborate effectively with a multidisciplinary team

How to Stand Out (Preferred Qualifications):
• Associate's Degree in a healthcare-related field (e.g., nutrition, counseling, social work)
• Bilingual skills based on community needs
• Familiarity with healthcare systems and community resources
• Knowledge of health education and social services
• Current Community Health Worker (CHW) Certification preferred
