RN Case Manager / Remote with One Field Day Location: Brooklyn, New York Job Type: Direct Hire Work Site: Hybrid Category: Case Management Skills: Salary Low: $108000 - Salary High: $115000 Duration: 2 Weeks Shift: Position: 145519 Job Description RN Case Manager / MLTC / NYC / Remote with one day a week in the field / Multiple positions available We are hiring multiple RN Case Managers to join a leading MLTC organization, with opportunities available in Bronx, Brooklyn, and Manhattan. Each RN will be assigned one borough and will primarily serve members within that area. This hybrid position includes remote flexibility with one field day per week for in-home member visits. RN Case Manager Position Details by Location: Bronx: Bilingual Spanish preferred Brooklyn: Bilingual Chinese, Spanish, or Russian preferred Manhattan: Bilingual Cantonese or Mandarin preferred RN Case Manager Key Responsibilities: Manage a dedicated caseload within one borough, ensuring members receive appropriate and accessible care. Develop, oversee, and update individualized care plans to align with members' evolving needs. Conduct home visits (one borough, 4-6 visits per week) to assess home safety and care needs. Collaborate with leadership and Intensive Care Management teams to address complex cases. Present high-risk cases for multidisciplinary review. RN Case Manager Qualifications: Active New York State RN License required. Bachelor's degree in Nursing, Health, or Human Services preferred. Minimum two (2) years of home care experience required. Strong experience in care planning, patient assessments, and home safety evaluations. Prior supervisory experience is a plus. Bilingual candidates strongly preferred (see language preferences by borough above). Open to training RNs with Chinese language skills. Proficiency in Microsoft Office.
