Geriatric Social Worker (Hybrid/ Remote Part time

Job description

Conduct assessments and evaluations to determine clients' needs and develop appropriate treatment plans
Collaborate with other healthcare professionals to coordinate care and ensure holistic treatment
Assist clients in accessing community resources and support services
Maintain accurate and up-to-date documentation of client progress and treatment plans

Minimum of [1] years of experience as a Geriatric Social Worker or related field (Care Manager, Case Manager, Discharge Planner/ Coordinator etc)

BSW, MSW, Psychology or Human Service degree (any related field )
Ability to navigate medical documentation systems and maintain confidentiality of client information
Experience working as a Discharge Planner in a nursing home or hospital is required
