What to Expect (Job Responsibilities):
• Identify community-based organizations as potential referral partners
• Develop relationships with referral partners and conduct presentations to educate staff on program services
• Provide face-to-face outreach, orientation, and screening for referred members
• Utilize telephonic and electronic outreach to obtain necessary information for enrollment
• Maintain accurate case records in designated systems
What is Required (Qualifications):
• High School diploma
• 1 year of relevant experience
• Bilingual in English and Spanish is preferred
• Excellent public speaking, interpersonal, and time management skills
How to Stand Out (Preferred Qualifications):
• Experience in outreach, enrollment, or community engagement roles
• Knowledge of health home programs and services
